Survey Request
The Department of Research, Data, and Evaluation (RDE) offers survey services for district-level employees and school-based administrators. There are four types of “surveys” the survey platform Sogolytics provides: surveys, polls, event feedback, and online applications/forms.
- Surveys gather information on a specific topic (budget, calendar, summer program).
- Polls gather responses from a single question (What color do you prefer?).
- Training/Event Feedback provides presenters or facilitators with information about the event they hosted, and the content presented (Summer Leadership Conference, Leadership Academies, New Teacher Induction Seminars)
- Online Application/Form replaces the traditional “paper & pencil” application or form with an online format (Registration, Sign in, Superintendent Advisory Council Application)
This form can also be used to submit a request for a focus walk form using the Grow by Level Data platform.
